Once we receive the form, we will verify insurance benefits and contact the family to schedule the appointment. If you choose private pay and are not using insurance, a physician’s referral is not required. We are able to accept referrals based on caregiver concerns.
Please contact our clinic directly to schedule your child’s assessment. After the evaluation is completed, a copy of the report will be provided to the physician and parent(s)/guardian(s) via email and/or mail.
