A Physician's Referral
Thank you for your referral! Below are the steps to submitting a referral for our services:
- Complete and sign our HIPAA compliant form
- Or complete and fax our paper form back to Columbia (931-490-7771) or Lawrenceburg (931-766-6433)
- We will check insurance then contact the family to schedule your appointment.
- After completion of evaluation, physicians and parents (guardians), will receive a copy of the report via email/mail and/or you may be added to our EMR Portal to view there.
